We are excited that you have chosen our office for your oral and facial surgery needs. if you have a referral from a dentist or family doctor please bring it to your consultation appointment. Please also bring any medical information and radiographs if you have available with you. We know that you are busy and want to spend the least amount of time possible at your consultation appointment, so we have made it easy for you to fill your new patient information sheet on our website. You can fill this information here and either submit it or print and bring it to your consultation.

Financial Information

At TOFS we work with all dental insurance companies. You are responsible for the full fee of your appointment. We do submit your dental claim to your dental insurance company electronically and you will be reimbursed by the insurance company. We accept Debit, Visa, Mastercard, and cash.
